My first visit with MS Poe was on 12/28/22. We had a discussion regarding my hypertension. I mentioned to her when I have elevated episodes I suffer headaches and blurriness of vision. She noted in the visit log: “ This is a chronic problem. The problem has been waxing and waning since onset. The problem is uncontrolled. Associated symptoms include blurred vision and malaise/fatigue”. I was prescribed amlodipine due to my suggestion and was scheduled a follow up on 2/22/23. During my second visit I reiterated what was going on with me and requested FMLA paperwork to be completed. MS Poe advised me to upload documents and she would complete. After paying an additional $25 to complete, I received the completed form with excusing one day a month and one appointment for the year. I requested to have the form amended due to the fact that we previously discussed the frequency of the episodes where more than once a month. My request was denied and I was advised to find a cardiologist
