Dr. David Maze, OD

My child was to have vision therapy after We paid a lump sum fee of nearly $6,000 to begin a series of therapies. The sessions were NOT covered by insurance; we requested help with the therapy codes to assist with insurance. The team at Dr Maze's practice claimed the codes were always accepted and had not been a problem with insurance coverage before. BCBSIL covered none of the expenses. We stopped therapy for 2 main reasons - my child had only minor improvement with the therapy kit we were sent home with, and, the in-office therapy times were difficult to schedule (openings were infrequent and tough to schedule around my child's school times). Dr Maze and his practice owe us nearly $5,000 and he has never responded, called, nor emailed us to get this matter resolved despite numerous attempts I made. His staff was hardly helpful, full of excuses. We feel robbed and very disappointed in our experience with Dr Maze and his practice in Illinois.
