Dr. Biesman failed to fully complete a medical source statement of ability to do work related activities for my spouse for disability purposes. Further, his assessment was not supported by any related testing, lab findings, or other medical evidence as suggested on the form. Not surprising really, when he spends about 15 minutes with the patient 4 or 5 times. How is it that he can determine whether someone can "understand and remember short simple instructions ..?"
