The office/nurse denied to let my kids have physical form for school try out. They said I HAVE to see the doctor so he can sign the form. And the appointment will be 2 months later. Tried to call to explain 3 times, request to talk with doctor but they just said, we'll call you back (which they dont). Dont recommend!
