Dr. Quililan is an excellent physician. Unfortunately his nursing staff is incapable of accomplishing a task as simple as sending a laboratory test request form in the mail. On multiple occasions I was required to fast over 12 hours for blood tests, then denied service because the nurses didn't provide the required documentation for blood tests.
