We decided to stop seeing Dr. Lesniak because our chart stated an inaccurate health condition. It also listed the need for medication. This must have been from another patient’s records and could be very dangerous misinformation. She was friendly but the mistake in records has us questioning the accuracy of vaccines received etc.
