Dr Lewis did not listen to what I was saying and wrote incorrect information in my chart. She also insisted that I was 'depressed' and put that in my chart as a diagnosis which was untrue. I have never been diagnosed or tested for depression. I had some health issues at the time and was upset at my condition but she insisted that I was depressed. I felt almost bullied by her that I was supposed to be depressed and later saw she had permanently diagnosed me as depressed on my chart based on our 15 min visit, even though my health condition was only temporary and I'm not depressed. I was going through a challenge at that moment and have never been diagnosed or treated for depression. After reading the notes in my chart, I realized that she wasn't paying attention to what I was saying and she wrote many untruths about me. I don't have faith or trust in her.
