NOTE: In order to send in this review, I was required to check the box on this form that I received treatment from this doctor . However, I was NOT TREATED YET WAS BILLED over $1500.00. In March of 2025 I went to the hospital after hours for a procedural drug test due to an accident related to my job. I peed in a cup, paid $90.00 for the test, results were sent to my employer and I had a zero balance. I did not see a doctor but was sent a bill for over 1500.00. I called the billing service in December of '25 and explained the situation. Nothing was resolved. I received another bill in the mail in January '26. I contacted the billing service again and they told me I was seen by Dr. John Keith and directed me to call the billing department at Tristar. Tristar told me that doctors are contracted and advised me to google the doctor's number. I did that and it led me back to the Tristar Operator. It all started again. There was no way to contact this doctor and dispute a bill I don't owe.
