Promises will not perform or schedule surgery without Pre-Auth as it is considered experimental by almost all US insurance companies (simple Google search will tell you that). Calls says she has pre-auth, then sends tens of thousands of dollars bill to patient well after appeal date and will not even consider working with patient on cost.
