She sent in my prescription wrong not once, not twice, but 5 times in a row. This is a medication that causes severe withdrawal when I do not have it. It is not an optional thing. She had multiple opportunities to refill it as she knew for days ahead of time that it needed to be refilled, she just didn't refill it and instead took care of other patients. It would have taken 5 minutes for her to send the prescription in. 5 minutes of her time with 3 days notice, while she was actively working and she just didn't do her job.This was repeated. She sent it to the wrong pharmacy twice, wrote it incorrectly twice, and sent in two conflicting prescriptions the other time. I was forced to use her as I was in addiction treatment and required to use their provider.The worst thing she did, though, was lie to me. She told me that after I was out of her care (they did not set me up with a new provider) she would still write a script and didn't, leaving me without medication. Awful.
