Saw Koyoda for the first time for worsening arthritis and carpel tunnel syndrome. He sent a request to my insurance carrier to pay for a very important apparatus to help with the pain and to help treat it. He filled out the form wrong and the carrier rejected it. I called him a number of times leaving messages how to correctly fill out the forms. He never got back to me. He never filled them out again. He never sent in a second request to my carrier for it to pay for the apparatus. I had to pay for it myself. I also called him repeatedly about two other very important matters. By the end of the second month, he still hadn't returned my calls. I cancelled my coming appointment with KOYODA. I will never let KOYODA have anything to do with me again
