Both his offices look very old and devoid of patients...his magazines were bleached from age. He didn't have any up to date equipment -it was all from the '70s. He forced me to leave ALL of my credit information so he could pay himself "In case Anthem doesn't reimburse me." I felt weird so I paid him $80 cash. He didn't have 5 bucks to give back for change. I had to ask for a receipt -the receipt had the wrong year on it. I received no bills containing codes I would require to be covered by Anthem. My co pay was supposed to be $20. He forced me to go to his other office 50 minutes away from my home because he couldn't email a prescription. I drove there, picked it up, it was hand written and illegible. I was given a claim form from Anthem to complete. He wouldn't provide the codes for the form. He said "codes?" as if it were a completely foreign concept. I had to cancel my credit card. Don't go here. Or if you do, don't expect a real Dr.
