I had a straightforward incapacitating injury. As my provider, he went radio-silent for weeks at a time on two occasions by both phone and MyChart in response to simple questions, then dubiously blamed his office staff at a subsequent appointment. He refused some documentation requested by my HR for reasons he was unable to explain, and the rest was rejected by HR for being done incorrectly, more than once. I didn't actually need anything else from him, so this was all very frustrating, and I ended up needing to find a different provider for these same tasks, so it was a big waste of time.
