He was able to get me an appointment fairly quickly. was told I was a depressed. Gave him a $20 copay, and he made copies of my ID and insurance card. I did not get a receipt. He called me the next day, said he does not take my insurance, I could pay him $50 more for him to send a brief summary from our discussion. I paid him because I didn't want to take off work & go through that again. He sent over my stuff a week later(said it would only take 2 days) and had my DOB wrong. Never again!!
