I expressed concerns to Dr Tartar of my father's cognitive status, including the ability to care for himself (take his meds, complete personal care, etc.), his safety in his home, his confusion including hallucinations & paranoia, she minimized these concerns in her report. She also minimized other health professionals' concerns about his need for 24 - 7 care & oversight. For example, his head MRI results which would have been available when she completed her evaluation, stated 'evidence of moderately extensive small vessel ischemic change & brain volume loss'. In addition, she also didn't include information about my father's performance on the Montreal Cognitive Assessment (MOCA) during his last hospitalization, which was an 8 out of 30 or severe cognitive impairment (a score of 26 or over is considered normal) As a result, my father was returned home from a memory care unit to live unsupervised for much of his days in his home.
